FILLING OUT YOUR PROTOCOL SUBMISSION DOCUMENT

Please click on the link below to access the Protocol Submission document.  After clicking on the link, you will see the file download prompt.  Click Save to download a copy of the Protocol Submission document to your computer.  Do not click open.  Make sure to save it onto your hard drive.  The document is in Microsoft Word format.  It will allow you to enter the required information directly from your computer keyboard into the form.  You may enter data into any of the grey fields which appear in the document.  Turn check boxes on or off with your mouse.  You can save your work at ANY time and return to it later.  Just re-open the copy that you saved to your PC.
