When will my Concordia Plan Service (CPS) benefits begin?
Enrollment will be effective the first day of the month following full-time employment.

What CPS Health Plan Cards will I be receiving?
Blue Cross Blue Shield of Minnesota and Express Scripts will mail cards to your home address via separate mailings directly from their company.
CIGNA Dental and CIGNA Behavioral Health card will be found in the Concordia Plan Service folder that is mailed to your home address.
Vision Service Plan and National Ear Care Plan do not require participant(s) to have a card to utilize their benefits.
Concordia Plan Services will also send a seperate mailing to your home address regarding the Concordia Retirement Savings Plan (CRSP) and the Supplemental Life Insurance Program.
When should I expect to receive my benefit cards?
The normal arrival time of the benefit cards is usually between 4 to 6 weeks of the date you complete your benefit enrollment with Concordia University Wisconsin.
How do I find a doctor, dentist, vision care specialist, or therapist?
Please visit www.concordiaplans.org > Select Who Am I? from the upper left hand side of the screen > Select Active Worker > Select FIND A DOCTOR OR DENTIST from the middle of the screen.

Click To find a doctor, (Blue Cross Blue Shield of Minnesota)
The identification prefix is 'CND'; you can then follow the prompts to locate a doctor in your area and/or search for a specific name.
Click To find a dentist, (CIGNA Dental)
Please select 'Dentist' on the top portion of the screen and follow the prompts for page 1. On the top of page 2 please select CIGNA Dental PPO (CORE NETWORK); you can then follow the prompts to locate a dentist in your area.
Click To find a vision care specialist, (VSP)
Please select 'Yes' when asked if you "Are You a VSP member?"; Proceed to sign on as a member or 'Register Now' if you have not yet registered; you can then follow the prompts to locate a vision care specialist in your area.

Please select 'Find A Provider' from the left side of the screen, you can then follow the prompts to locate a provider in the plan.
How do I submit my long term prescription(s) to Express Scripts?
(as taken from the Express Scripts website)
To fill a new prescription using Home Delivery:
First, have your doctor write a prescription for up to a 90-day supply of your medication (or for the maximum days supply allowed by your benefit). The prescription should include refills for up to one year, if appropriate.
Next, print a prescription order form from the Express Scripts website.
Lastly, submit your order by fax or mail using the instructions below. These instructions are also on your order form.
Note: If you have a credit card or check card on file with Express Scripts, your card will be billed automatically.
Order by Fax (Doctor Only)
Have your doctor or your doctor's staff fax your completed order form to 1-800-636-9494. Faxes must be sent from your doctor's office. Faxes from your home or workplace cannot be accepted.
Order by Mail
Mail your completed order form and your written prescription to the address below. Please write the patient's name, ID number, address and date of birth on each prescription.

After receiving your order, Express Scripts will process and ship the order in three to five business days. Please allow additional time for postal service delivery. For your protection, Express Scripts require a doctor's signature on all prescriptions.
Am I covered while traveling abroad?
While traveling outside the United States, members and their dependents enrolled in the Concordia Health Plan are covered for any emergency or urgent care not anticipated before leaving the U.S. Should you need medical care, simply go to the nearest hospital. Unfortunately, you will have to pay the full amount for any charges while there. However, upon returning back to the United States, send the bills to your network provider (Blue Cross Blue Shield) for proper reimbursement. All network copays and coinsurance will still apply as usual (those would simply be deducted from the reimbursement amount). For correct reimbursement, you should provide the foreign exchange rate applicable on the date the services were rendered.
For long-term prescription needs, you should get your prescription filled before you travel. If you require medication while abroad, you will have to purchase the medication and then submit an Express Scripts Out-of-Country Claim Submission form upon your return to the U.S.
You will need to include a detailed copy of the prescription receipt, any translation you can provide (if the original receipt is in a foreign language with foreign currency), and the currency exchange rate.
What form do I have to complete in order to enroll in (or to make a change to) the Concordia Retirement Savings Plan (CRSP) 403(b)?
A Salary Deferral Agreement Form will have to be completed and returned to the Human Resources Department.
This form allows you to begin a new enrollment or to make any changes to your current payroll deduction.
Note for New Enrollees: Please make sure you also visit the MetLife Retirement Savings Link which will allow you to name your beneficiary and to select investment options for your future contributions.

Do I have tuition waiver benefits and if so where can I find the policy?
Tuition waiver policy is available to all full-time* employees and adjunct instructors of Concordia University Wisconsin and their eligible dependents, if applicable.

*Note: A full time employee is any employee who is scheduled or contracted to work 40 hours per week.
