Degree Seeking Students

Please complete the Online Application and submit the $50 application fee online.

In addition to your application, please submit the following:

• One page essay as to why you would like to enroll in the graduate program that you have selected.
• Resume
• Two recommendations that are academic or professional in nature using the Recommendation Form, or recommendation letters on letterhead may be submitted.
• Official transcripts for all previous college experiences. Transcript Request Form

Applicants for the Physical Therapy program should also submit:
