CUW is encouraging all students to make sure they have adequate health insurance coverage. It is true that we have an excellent urgent care clinic on campus that is equipped to deal with acute illness, minor injuries, and chronic/stable conditions. However, our hours and services are limited. The cost of healthcare off campus can be quite costly and unaffordable without health insurance coverage in the area.
We are pleased to be able to offer a quality, low-cost plan, Student Assist, underwritten by Markel Insurance Company in Virginia, and administered by Pioneer Management Systems in Massachusetts. It is essentially a $50,000 basic accident and illness policy that costs $842 annually and provides coverage 8/1/07-8/1/08. Student Assist also provides Wellness Resources that offer discounts (not health insurance) at certain health care providers (Chiropractors, Dentists, Audiologists, Optometrists), pharmacies, mail-order vitamins & nutritional supplements. They also offer limited travel and roadside assistance, financial/legal telephone consultation, and a nurse’s hotline. You must consult the policy directly to find out exactly what is covered, what is excluded, and what may be purchased at a discount. We are requesting that all full-time equivalent undergraduate and all international students enroll or waive out of coverage on-line by 9/11/07. This is a managed enrollment process that we are using to gather information to make our insurance plan even better in the future! Please be patient with this process since this is our first attempt! Also, realize that the forms are generic and we could not change some of the verbage. Your tuition account will not be automatically billed if you fail to complete the online form! But we will keep requesting that you do so in order to gather statistics on how many students are adequately covered. Please log onto www.studentassist.com and follow the instructions using the password 0660 or 0663 and the policy # 07200663. You do not need to share your SSN, only your F00# (make sure to use the numerals 00, not the letters "OO"). If you waive out of the insurance, you should receive a confirmation # to print out. If you enroll, your coverage should be effective once you've completed the process on the web. Coverage begins 8/1/07. Insurance cards will be sent directly to you after 9/11/07 at the address that you provide when you enroll. Until then, you can obtain a claim form through www.studentassist.com and utilize CUW's policy # 07200663. We are also working on a process for obtaining a temporary card if you so desire.
Any full-time equivalent CUW student attending classes at the Mequon campus is eligible for enrollment in the Student Assist plan for 07/08, but must do so by 9/11/07. Spouses and dependents are also eligible ($1,888 for spouse and $1,263 for all dependents annually). An enrollment form can be accessed on-line at www.studentassist.com. Students should enroll on-line and will be billed via their tuition account. An enrollment form and payment will need to be mailed directly to Markel Insurance by 9/11/07 for spouses and dependents.
"If you need temporary health insurance, go to www.smartstm.com or call 1-800-279-2290. It takes about 10 minutes to get a quote, review your options and purchase the insurance online. It can be purchased from 1 to 12 months or anything in-between."
