The following immunizations are offered:

***Vaccine cost may be reduced for anyone 18 years old and younger that has no insurance or is without vaccine coverage under their insurance.
Contact Angie Palese or call x4574 (Health Services) with questions or to request a shot. Please come anytime during the designated time of the clinic - no appointment necessary - but let her know you are coming! Make checks payable to Concordia University Wisconsin or F00 account will be charged.
Call the Ozaukee County Public Health Dept. to schedule an appointment in Port Washington if the CUW clinic dates are not possible:
