There are two application forms that prospective students can print from the CUW web.
For faster processing, the applicants are advised to have all required documents ready, and mail all the documents at one time, in one envelope to the following address:

Check list of the documents the applicant should mail to the school:
- Application form.
- Application fee, $35.00 U.S. dollars for graduate admission; $50.00 U.S. dollars for undergraduate admission, in money order or cashier's check..
- Official transcripts from the schools the applicant has attended. If the transcripts are not in English, have them translated into English and notarized.
- Applicant's brief resume and biography. Please use a typewriter or a computer.
- Two (2) reference letters from the teachers the applicant has had, commenting on the applicant's ability to finish the degree.
- A short essay, stating the purpose of pursuing the U.S. degree. (Graduate applicants only).
- A bank statement with the balance converted into U.S. dollars. If a relative or parent is sponsoring the applicant, have that person write a letter of affidavit, and provide with that person's bank statement. If a company is sponsoring the applicant, have that company write a letter of affidavit, and provide a financial statement of the company on company letterhead.
- If the applicant is already in the U.S., please enclose copies of I-94, passport, visa type, such as B-1, B-2, J-1 (DS2019), F-1 (I-20).
It usually takes 4-6 weeks to finish processing the application. If the applicant is accepted, an acceptance letter and an I-20 form will be mailed to the applicant. If the applicant is denied, a letter of notification will also be sent to the applicant.
English proficiency requirements: we require TOEFL 500 on paper, 173 on computer for undergraduate applicants; TOEFL 550 on paper, 213 on computer for graduate applicants. Undergraduate students who do not have TOEFL score or whose TEOFL score is low, can be accepted into English as a Second Language programs.
