About Ascension Wisconsin Pain Management

The Ascension Wisconsin Pain Management team is an interdisciplinary collaboration of healthcare providers providing pain management services through a multimodal treatment approach. The team consists of two lead physicians (anesthesiologist and physical medicine and rehabilitation specialties), one physician assistant, one pain psychologist, and one clinical pharmacist within the Center for Pain Management. Services are provided at four different Columbia St. Mary’s locations throughout Milwaukee and Ozaukee counties. Through this collaboration our patients receive a comprehensive treatment approach for their chronic pain including interventional analgesia (i.e. epidural steroid injections, radiofrequency ablation, etc.), physical rehabilitation (i.e. PT, OT), chronic pain self-management education/training, and comprehensive medication management (CMM). Our current CMMA project is focused on implementing pain medication education into our 8-week self-management education/training groups, as well as integrating the clinical pharmacist into the care team as a physician extender.

About Ascension St. Joseph

The Ascension St. Joseph Professional Office Building 6th Floor Family Medicine team is rooted in the loving ministry of Jesus as healer.  Our team consists of three physicians, two nurse practitioners, one physician assistant, nurses, social worker and clinic supervisors.  We commit ourselves to serving all persons with special attention to those who are poor and vulnerable.  Our Catholic health ministry is dedicated to spiritually centered, holistic care which sustains and improves the health of individuals and communities. We are advocates for a compassionate and just society through our actions and our words.   The Ambulatory Care Pharmacy Services provide medication therapy patient care services for adult patients who receive medical care in this clinic.   The pharmacist provides patient care services to all patients requiring medication therapy, regardless of diagnosis or acuity. Major conditions most often treated include the following:  Diabetes, Hypertension, Hyperlipidemia, Anticoagulation, Asthma/COPD, and Heart Failure.    The primary focus of the team has been to develop sustainable, integrated pharmacy services with the goal to decrease A1c to <8% for 84 patients with diabetes and move one provider from 25th to 50th percentile of patients meeting A1c goals between February-November 2018.

About Aurora Health Care

The Aurora Health Care CMMA team project focuses on the creation of an adherence program for our refugee patient population. The Aurora St. Luke’s family medicine clinic sees a large volume of refugee patients. Providers from the family medicine clinic have identified refugee patients as higher risk for potential medication non-adherence due to low health literacy, transportation issues, and differences in culture and language. To overcome these challenges, an adherence program was developed that involves three pharmacist-led services including, comprehensive medication reviews, medication synchronization, and adherence packaging.  An initial pilot of the adherence program is being performed in the Aurora Pharmacy located in the Aurora St. Luke’s Physician Office Building, one floor below the family medicine clinic. Through the adherence program, our goal is to improve health literacy, medication adherence, and clinical outcomes for our refugee patient population.

About Evergreen Pharmacy

Evergreen Pharmacy, located in West Allis, Wisconsin, is an independent specialty pharmacy with a focus on patient centered care. We collaborate with patients and healthcare professionals to create customized solutions for each care plan. We are always adapting our services to fit the needs of the patient and their care team. In addition to pharmacy services, we offer medication synchronization (MedSync), an onsite injection clinic, and telepsychiatry. We also collaborate with health care providers to deliver patient education, and administration of initial/maintenance dosing offsite through local clinics. Our service areas of focus include: Rheumatology & Immunotherapy, Behavioral Health, Hepatitis, Methotrexate, Cardiology, Osteoporosis, Gastroenterology, and Dermatology. 

Our CMMA project is to grow and expand our MedSync program with the goal of increasing patient and provider satisfaction by increasing patient adherence to medications.

Hayat Pharmacy: Medication Therapy Management Team

Hayat Pharmacy is a Milwaukee-based independent pharmacy with over 10 locations. Hayat Pharmacy is focused on patient adherence, in-person medication therapy management (one-on-one medication education with a pharmacist) and medication synchronization through our Simplify My Meds program. Our pharmacist work collaboratively with other members on patient's healthcare team. Hayat Pharmacy is a nationally-recognized leader in the field of innovative medication management. We are dedicated to improving the health outcomes of patients with chronic conditions. Our award-winning adherence programs and motivation to implement optimized programs to meet our patients' needs while delivering unmatched local patient fulfillment.

Our CMMA project aims towards expanding our existing Diabetes Self-Management Education (DSME) program and our target population is primarily patients with uncontrolled Diabetes on multiple medications. We serve a wide age group of patients who are diagnosed with Diabetes. We work with patients who are dual eligible through Medicaid and Medicare, Medicare patients who have low health literacy so our goal is provide DSME in utilizing visual tools and following up with our patients.

About Marshfield Clinic Health System

Marshfield Clinic Health System is the largest private group medical practice in Wisconsin and one of the largest in the United States, with more than 1,200 medical providers representing more than 86 specialties and subspecialties. The System has more than 50 locations in 34 Wisconsin communities in northern, central and western Wisconsin, including hospitals in MarshfieldEau ClairePark Falls and Rice Lake.

The CMMA team consists of pharmacists from around the system, the pharmacy director at the wholly owned health plan, the pharmacy director, pharmacy technicians, and pharmacy data analyst.

The CMMA project is a collaboration with the health plan to improve medication utilization in high risk patients in the high risk commercially insured population. The goal of the project was to demonstrate that pharmacist using CMM can reduce the overall cost of care in this population.

About Monroe Clinic

Monroe Clinic is an integrated health system consisting of a 58-bed hospital and a robust multi-specialty clinic. We are located in a rural community and service 29 counties throughout southern Wisconsin and northern Illinois. Our project’s goal is to improve access to primary care services for our patients. We accomplish this by incorporating Pharmacotherapists into the primary care team to collaboratively manage patients with chronic conditions.

Reedsburg Area Medical Center (RAMC) Diabetes Care Program

About Reedsburg Area Medical Center (RAMC) Diabetes Care Program

RAMC is a 25-bed critical access hospital with general and specialty clinics, a nursing home, assisted living facility, and two retail pharmacies. RAMC is self-insured.

The goal of the CMMA project is to reduce overall cost of care for pre-diabetic/diabetic patients. Our value proposition to our customers is using evidence-based diabetes medical management and self-care tools to help them live longer, healthier and have fewer complications by formulating healthy behaviors.

Program components include:

  • Diabetes empowerment education program - six 1.5 hour sessions
  • Health and fitness coordinator - monthly visits
  • Dietitian – 1-2 visits per year
  • Diabetes Educator – 1-2 visits per year
  • Pharmacist – quarterly visits, comprehensive medication management (CMM)
  • Provider – annual visit

Our Diabetes Care Program (DCP) Goals:

  • Goal weight loss of 5% in 12 months
  • A1c goal of less than 7.0%
  • Blood pressure less than 130/80 mmHg
  • Reduce your risk factors for heart disease
  • CMM - right medication for the right indication, adherence management, medication safety and effectiveness review
