Launch Event - May 19, 2017

CMMA is launching at a live event on May 19th at Concordia University Wisconsin and open to all those wishing to explore participation in the CMMA initiative. Conversations will focus on community-based solutions, partnership development and CMMA team formation while introducing two “accelerator” tracks to kick-off in summer 2017. Accelerator tracks offer strategic development support and monetary awards for teams to implement CMM innovations. Attend to learn how you may play a role in this exciting initiative. There is no fee or obligation to attend!

CMMA Participation

Who can participate?

This CMMA initiative encompasses all stakeholders across Wisconsin committed to improving medication use for the Triple Aim of better care, better clinical outcomes and lower costs. Participants will join or form teams to then embark on a transformational journey. Teams may be based in clinical operations, cross organizational arrangements or policy level groups. Participants can include:

  • Providers: Health care organizations, pharmacy, and community service partners across the state of Wisconsin
  • Stakeholders: Community leadership, businesses, payers and health policy makers Teams do need to include a pharmacist, physician and a quality improvement and/or organizational administrative leader.

CMMA Accelerator Tracks

Interested participants and their team have the opportunity to participate in one of two Accelerator Tracks. 

  • Service Accelerator (for programs with an operational track record)
    • This may include: Health care organizations, primary care clinics, health systems and/or pharmacies working to advance or adopt CMM within their organizations may participate in this track.
  • Start-up Accelerator (for one organization launching ambitious CMM programs or initiatives)
    • Wisconsin-based start-up businesses or organizational business units seeking to offer programs that will provide CMM services may participate in this track.

Both tracks will support development of team strategies to integrate comprehensive medication management services into: (a) primary health care delivery for patients with chronic illnesses and (b) care transition programs involving primary care, hospital inpatient, skilled nursing facilities, and home care services.

Participation in these tracks will put teams on a path to compete for monetary awards at an accelerator event in fall 2017. These awards are $5,000 to $12,500 for the Service Accelerator and one $20,000 award for the Start-up Accelerator.

The awards are made to acknowledge the power and promise of the leadership pitch. The team can use the award as it sees fit. With the award the teams will be asked to play a leadership role in creating the market for CMM in Wisconsin.

Participants in both Accelerator Tracks will have the opportunity to pitch their innovations to a panel of judges at a live event in fall 2017.

Launch Event Materials

On May 19, 2017, 55 individuals representing 27 organizations across Wisconsin attended the CMMA Launch event at the Concordia Center for Environmental Stewardship. Participants reviewed the urgent need and possibilities to improve health and medication use in Wisconsin through comprehensive medication management. An overview of the service and start-up accelerator was provided, and the activities to be completed over the next 18 months in each were reviewed. Participants were also introduced to the business model canvas, a business model development tool that will be utilized by all participants to formulate the focus and process for their initiative.

1. What is CMM?

2. Should I participate in the service accelerator track or start-up accelerator track?

Simply stated, the following summarizes which track a team should participate in:

Service-Accelerator Track: If you are currently working in a health care organization and desire to newly implement or improve CMM within the structure of that organization, the service-accelerator track is for you. Teams, at a minimum, must include a pharmacist that is or will provide CMM, a physician champion, and another key stakeholder such as quality improvement professional, information specialist or executive leader.

Start-up Accelerator Track: If you are interested in developing a business entity around an idea/innovation that supports CMM or provides CMM to patients as a newly formed business, the start-up accelerator track is for you. There is no team make-up requirement, but you must be developing a new business in WI or have an existing WI business with less than 5 years’ experience.

3. What will the next 18-months of the CMMA look like?

The next will consist of two phases.

Phase I

The remainder of 2017 will focus on business development coaching for both tracks. All teams will participate together in monthly webinars on business model development and CMM. Each teach will have monthly scheduled coaching calls. A “mock-pitch event” will be offered live or virtually to all teams in September. All this will culminate in a live pitch-event in November 2017 in Milwaukee.

Phase II

For the ten teams accepted into the service accelerator track and the one team accepted into the start-up accelerator track, 2018 will focus on implementation. The two tracks will differ in activities with the service accelerator teams continuing to participate in monthly webinars on practice implementation and pacing. Teams from both tracks will have monthly scheduled coaching calls. Live events at mid-point (June) and conclusion (November) will take place in Mequon, WI.

4. What are we going to be pitching in November 2017?

5. What will this cost?

Participation is free and covered by a gift from Dohmen companies. Teams that are invited to continue in each track after November may receive monetary awards.
