Celebrating 18 Months of Performance

The Concordia Medication Management Accelerator (CMMA) Performance Story Symposium will highlight the efforts of health care teams and start-up businesses around the state of Wisconsin advancing the practice of comprehensive medication management (CMM). Teams participating in an 18-month learning and action collaborative will share the results of their efforts to improve the health and medication use of the patients they serve. 

In addition to the sharing of performance stories, keynote speakers will provide insight and participants will engage in conversations to leave in action to further the spread and sustainability of CMM.

When: November 16, 2018 10:00am – 4:00pm CST

Cost: Free

Health care professionals, faculty members, health care payers, innovators, government officials, entrepreneurs, and anyone interested in enhancing the value of health care through the safe and effective use of medications are invited to attend. 

There is no limit to the number of individuals who may attend from one organization. CMMA teams presenting are encouraged to bring their full implementation team, including executive leadership.  

Each individual from an organization planning to attend the live event is requested to submit an RSVP.

Meet the Keynote Speaker

Tom Evans, MD, FAAFP

President and CEO

Tom Evans, M.D. is the President and CEO of the Iowa Healthcare Collaborative (IHC). The Collaborative focuses provider-led initiatives to promote transparency, improve clinical performance, improve patient safety and reduce cost. Dr. Evans received both BA and MA degrees from Drake University. He received his MD degree from the University of Iowa in 1983 and completed his Family Medicine residency at Broadlawns Medical Center in 1986. He practiced family medicine for 13 years, and served as Chief Medical Officer for the Iowa Health System from 1999 to 2005. He has served as President of both the Iowa Medical Society and the Iowa Academy of Family Physicians. He has also served on the board for the National Patient Safety Foundation, and on the delegations for both the American Medical Association and the American Academy of Family Physicians. Dr. Evans currently serves as a faculty member with the Institute for Healthcare Improvement, Des Moines University, and the University of Iowa College of Public Health.
