Application Materials

Cover Letter: Provide a cover letter that outlines your interest in the Concordia University Wisconsin residency program and highlights attributes that support your candidacy for our program. You’ll want to describe why you’re interested in each particular site. For the 2016–2017 cycle, applicants will have the option of submitting an individual cover letter to each site or one general cover letter that includes one paragraph describing why you’re interested in each particular site. No preference is given to individual or combined letters. Please use the format you feel will most benefit you and your application.

Curriculum Vitae

Three letters of reference (minimum): Reference writers are encouraged to submit a letter in addition to completing the standard form available through PhORCAS. Reference writers should submit program-specific letters in the text box provided in the reference portal standard form, either by copying/pasting or typing a new letter. PhORCAS no longer supports uploaded letters. Please don’t have reference letters mailed to our program, as they won’t be considered.

Transcript(s): Applicants should mail one copy of the most up-to-date pharmacy school transcript(s) to PhORCAS at: PhORCAS Transcripts Department, P.O. Box 9124, Watertown, MA 02471.

Eligibility: Doctor of Pharmacy degree (Pharm.D.), eligible for immediate licensure by the Department of Professional Service.

Additional Requirements: A Program Specific Application is required of anyone applying for the 24-month option (required supplemental documentation for PhORCAS) located here.  Upload and submit the completed form along with your application through PhORCAS.

Non-U.S. Citizens

We’re currently accepting applications from non-U.S. citizens, but we’re unable to sponsor visas for residents. In order to apply, a candidate must be able to secure an appropriate visa for employment by Concordia University Wisconsin, as well as ensure they’ll be qualified to take the licensure examinations by June 2017.

Selection Criteria

Residents are selected based on their commitment to patient care, as well as their dedication to advancing the profession of pharmacy. Strong verbal and written communication skills are essential. Demonstrated involvement in a variety of professional activities and potential to express leadership in the profession are highly desirable.

Interviews and "The Match"

Residents submit one application to our program, which is reviewed by the preceptors at each site to which the candidate has applied. Sites then select the candidates whom they wish to interview. For example, a candidate may apply to two sites in our program, and then be invited to interview at one or both of those sites.

Each residency training site has its own listing in the residency matching stem, so candidates rank each site with which they interviewed in our program individually within their overall rank order list. Thus, a match with our program creates a match directly to a specific affiliated site.

Please see our Residency FAQs page for questions on the residency application process.
