At Concordia’s Interprofessional Clinic we prioritize under-insured and under-served populations in Southeast Wisconsin and partner with local agencies to meet/address health and social concern priorities in our region.
Emphasis on patient-centered care
Patients/clients are members of the team and the reason the team exists. Shared decision making and patient/client preferences are highly valued as the basis for the patient/client and provider relationship. Nihil de nobis, sine nobis: “Nothing about me without me”. This also includes the “client as teacher” concept.
Priorities for care
The CUW Interprofessional Clinic was designed to help meet unmet community health and social care needs when the provision of that care is not in direct competition with other community providers. To help preserve positive community provider relationships; while providing needed care to underserved populations, the following guidelines will be followed when determining if a potential client will receive care in our clinic.
Care will be prioritized for individuals who:
- Have no insurance
- Exhaust insurance reimbursement for identified and needed services
- Demonstrate prognosis for ongoing improvement
- Meet other guidelines
Care will not be prioritized for individuals who:
- Have other community options for services that are covered by their health insurance
- Receive duplicative health or social care services covered by their insurance, which are currently meeting their needs
Patient rights and responsibilities
Patients' rights include being treated with dignity, involved in their care decisions, receiving clear information, and having their privacy protected, while responsibilities include providing accurate information about their health, cooperating with treatment, keeping appointments, and being respectful of others and the healthcare facility. These rights and responsibilities help foster a respectful and effective patient-provider relationship, ensuring quality care and promoting patient safety.
